LPN Intake Coordinator Job Summary

A qualified Licensed Practical Nurse Intake Coordinator assumes daily responsibility for receiving intake information, assists with processing referrals, coordinates home health care services within the branch office and when needed, administers skilled nursing care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Registered Nurse. Services are furnished in accordance with Agency policies.

Qualifications:


  • Graduate of a state-approved school of practical (vocational) nursing and currently licensed in the state(s) in which practicing.
  • Minimum of one (1) year experience in nursing, home health preferred.
  • Acceptance of philosophy and goals of this Agency.
  • Ability to exercise initiative and independent judgment.



Responsibilities:


  • Understands and adheres to established policies and procedures.
  • Receives intake information and routes it to the supervisor for admission approval. Enters initial intake information into Agency software as needed to ensure timely scheduling of admission visits.
  • Communicates with clients/families/physicians and other referral sources on referral, care, and service delivery issues in a professional, caring, and competent manner as required by the customer service policies of the Agency.
  • Ensures care coordination via daily communication with all healthcare team members.
  • Ensures the delivery of appropriate and physician-ordered equipment and supplies as needed. Assists with inventory as needed.
  • May assist with the scheduling process and daily coordination of visits for the field clinicians.
  • Performs audit of clinical notes daily for completeness, skill, and medical necessity within 48 hours of clinician submission.
  • Receives labs, reviews, and forwards to physicians as necessary. Follows up to ensure orders are received in a timely manner. Prioritizes critical lab results and implements a system to ensure these are communicated to physicians immediately.
  • Performs skilled nursing visits as assigned within the limits of preparation and only provides those treatments for which he/she is qualified through licensure, competency, training, and experience.
  • Evaluates with the registered nurse the effectiveness of the LPN’s nursing service to the patient and family under the guidance of the registered nurse.
  • Documents changes in patients’ plans of care daily via verbal orders.
  • Performs clinical record review audits as assigned.
  • Attends staff meetings, patient care conferences, and in-services as scheduled.
  • Takes on-call duty, nights, weekends, and holidays as assigned.
  • Is responsible for:
  • Submitting any changes in schedule to the Director of Clinical Services/Nursing Supervisor on a daily basis.
  • Participating in patient care conferences to discuss the need for the involvement of other members of the health team, such as physical therapists or speech-language pathologists.
  • Prepares clinical and journal notes. Submits required notes to the branch office per Agency policy of twenty-four (24) hours.
  • Assists with Performance Improvement data collection and other activities as needed and at the request of the Clinical Manager.
  • Demonstrates acceptable attendance and punctuality.
  • Follows directions of the Clinical Supervisor and Clinical Manager and is supportive of branch, Agency, and management policies and procedures.
  • Demonstrates professional and positive attitude at all times with supervisors and peers.
  • Demonstrates constructive conflict resolution skills and utilizes the appropriate chain of command to resolve problems/issues.
  • Maintains a professional appearance at all times and follows applicable uniform policies (ID badge, uniform).
  • Performs other job duties as assigned.



Working Environment:

Works indoors in Agency office and patient homes and travels to/from patient homes.



Job Relationships:


Supervised by: RN/Clinical Supervisor/Clinical Manager/Director of Nursing/Administrator.

High risk.

Physicians, nurses, and other health professionals may access any information contained in a patient’s records (other than information restricted by a patient-provider) for the purpose of treating the patient, including consulting with other professionals concerning the patient’s treatment.
