RN 

Job Summary

A Registered Nurse administers skilled nursing care to patients on an intermittent basis in their place of residence.  This is performed in accordance with physician orders and plan of care under the direction and supervision of the Clinical Manager. Services are furnished in accordance with Agency policies and procedures.

Responsibilities:

 

  • Provide services in accordance with the physician approved plan of care

  • Make the initial evaluation visit and regularly reevaluate the patient’s nursing needs

  • Initiate the plan of care and make necessary revisions

  • Supervise LPNs and certified nursing/home health aides

  • Provide comprehensive teaching to patients and their caregivers

  • Submit accurate patient documentation per Agency policy

  • Provide those services requiring substantial specialized nursing skills

  • Initiate appropriate preventive and rehabilitative nursing procedures

  • Inform personnel of changes in the condition and needs of the patient

  • Counsel the patient and family/significant others in meeting nursing and related needs

  • Attend in-service programs as required

  • Understand and adhere to established Agency policies and procedures

  • Process orders and notify physician of patient needs and changes in condition

  • Complete certification/recertification orders and discharge summaries

  • Determine the amount and type of nursing needed by each individual patient

  • Refer to Physical Therapist, Speech Language Pathologist, Occupational Therapist and Medical Social Worker those patients requiring their specialized skills

  • Conduct patient care conferences on patients assigned to his/her care

  • Take on-call duty nights, weekends and holidays, as assigned

  • Complete and submit assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition in accordance with Agency defined time frames

  • Perform other duties as assigned

Qualifications

  • Graduate of an approved school of professional nursing and currently licensed in the state of Tennessee

  • One (1) year of nursing experience required

  • Acceptance of philosophy and goals of this Agency

  • Ability to exercise initiative and independent judgment

  • Ability to communicate effectively, both verbally and in writing

  • Ability to turn, lift, carry and otherwise transport patient

  • Current CPR certification and automobile liability insurance

  • Willingness to work in home environments of patients who may have values or standards different from your own
